What’s the first sign that a software project has outgrown

Lately our small app team has been adding more people almost every month, and I started noticing that simple decisions suddenly need three meetings. A year ago two developers handled everything and it felt easy. What was the first sign for you that a software project had simply become too big for its original team setup?
